So the rules state that a when "the first time coming within 12 inches and los of a building" to create a PEF.
Does this mean that the first time each building is approached, it needs to be rolled as a potential PEF.
Or just the very first building that is approached is rolled only as a PEF.

I am no rules authority, but I understand it to mean to roll for each building.
In my experience, it is good to exercise some judgment with this rule, or you might easily get massive enemy forces. For example, when approaching a farm with main building, barn, stables etc. I just roll once for the whole farm and place the PEF in the most likely-looking building. And if you are refighting Stalingrad...

Each building, but can be adapted as desired. It's your game. It makes you think about from which direction to approach buildings, never more than one at a time if possible.
